Improving the quality of health in Barnet through sustainable primary care

Anticoagulation

Barnet Federated GPs were awarded the provision of the Community Anticoagulation Service. Patients registered in a GP Practice within Barnet are referred to this service by their usual GPs.

This service commenced in April 2019 and is Pharmacist led community service which is responsible for the monitoring and management of people who take oral anticoagulant drugs. The aim is to work with patients, carers and other health care professionals to improve the safety of these patients.

  • PHGH Doctors, Temple Fortune Health Centre, 23 Temple Fortune Lane, London, NW11 7TE (Mondays)
