Introduction and purpose of the job

The Expanded Supporting Families is a government led initiative that encourages services to identify, work with and turn around families who have met criteria for attachment to the SF Programme.

Working under a matrix of management and supervision across Health Services and Children Services, the post holder will, play a key role in engaging individuals attached to the expanded SFP (under adult/child have a range of Health problems criteria). They will either act as the lead professional or identify a Lead professional within their organisation for cases which fall under the Supported Families criteria and where the primary concern centre around Health.  

The post holder will build a profile across the borough to presenting health needs and contribute towards multi agency discussions and planning within advisory boards and other strategic forums. 

The post holder will have oversight of Social Prescribing Link Workers, The Children and Young People Social Prescribing Link Worker (SPLW) will connect children and young people into a broad range of services and interventions in their local area in a timely and effective way, led by the wants and needs of the young person and their families.
